He suffered from OCD. Obsessive Compulsive Disorder. Those are usually seriously afraid of bacteria and wash their hands until they are red.
Back then people often viewed such people as "mentally ill" or plain "weird". Had Hughes not been rich it is possible they could have tried some rather controversial treating methods on him.
Even today people suffering from OCD are seen by ignorant people as "nutjobs", which is cruel and unfair.
