What the dealer did was inexcusable, as her hand was clearly not far enough out from her to suggest a fold, and she also did go all in on the hand (which the dealer should have noticed). The dealers at competition-level games especially are supposed to -not- be ignorant of the gameplay at their table.
I've had something like this almost happen to me. The dealer just looked at me and said, "fold?" ... I was like, "uh ... no" and she says, "protect your cards". Like wtf. I gave absolutely no indication that I was going to fold. My cards are up against the railing and nowhere close to the line.

I have seen a dealers push the pot to the wrong player, draw cards before the betting round was over, etc. Never assume the dealer knows what they are doing, they usually do, but not always.

This is mostly the player's fault. She pushed her stack forward, but didn't make a verbal declaration and didn't protect her hand. It is partly the dealer's fault for not paying attention, but the dealer can't see everything on the table at once. It's the PLAYER'S responsibility to always make their intentions CLEAR and to PROTECT THEIR HAND. In this case, the player did neither. It is sad, and the dealer is not completely innocent, but the player clearly takes the brunt of the blame here.

France: many political parties. When someone is corrupt they tend to face judicial consequences. You can freely protest. Very slow and (I heard) bad judicial system. Food is expensive.
US: 90%+ corrupt politicians and a lot of liars. OWS treated like garbage. Shitty judicial system even the Supreme court of justices corrupt. SOPA attempt. Lots of people watch Fox... I meant Fake shitty News. Food safety is corrupted by companies.

This is mostly the player's fault. She pushed her stack forward, but didn't make a verbal declaration and didn't protect her hand. It is partly the dealer's fault for not paying attention, but the dealer can't see everything on the table at once. It's the PLAYER'S responsibility to always make their intentions CLEAR and to PROTECT THEIR HAND. In this case, the player did neither. It is sad, and the dealer is not completely innocent, but the player clearly takes the brunt of the blame here.
