no that happens all the time. the box needs to respawn because of your settings, and the one on top falls. then when the other box spawns it spawns in the other box. to fix it try setting the run time minnimum to the maximum in the advanced forge settings. its an easy mistake to make

The camera was too close. If the camera is too close to the cannon, it won't show the blue junk. I've experimented with my friends doing this splitscreen, and from two different angles it is there and it is not at the same time.

Most likely the bubble shield didnt kill you. Before the bubble shield is thrown you hear a faint explosion...like the firing of a cannon. Thats the thing he was standing in front of,and he was killed by it. The thing periodically fires in the map,so its not the bubble shield.
