Maybe it causes him mental pain? Phycological pain? Or maybe they're talking about the person being cut and not the person cutting? Whatever the case, the real question is this: How exactly did he get past the rib cage to dig around for the key anyway?

I mean, the rib cage is just that: a cage. It takes more than just a scalpel to get through it. Or maybe I'm wrong and he was digging somewhere else?
