The whole commercial is done with obvious visual effects. There are really people who suffer under such disabilities and this commercial turns into poking fun and making business out of it. I'd call this rather low.

I didn't noticed something about the bulgarians, but you have to know that they have the best shirts. They are so interesting before there were shops girls used to saw their shirts and sometimes it tooked over 20 days to finish a shirt.
