how did greenlee die? and what happened to annie?

Ryan saw Zach and Reese kissing before the wedding and told Greenlee they were sleeping together. Greenlee didn't believe it and rode off (in her wedding dress) to find Zach and set the record straight. She was run off the road when Kendall (who was arguing w/Zach) swerved around a corner.
As for Annie - Ryan happened to her. She is currently in the nut house where Aidan is trying to help her get better.
