You fail to mention the regulatory standards set in place to ensure that the insurance companies do in fact represent accurate costs based on the level of representation they have. They can't just charge everyone whatever they want. They have to prove the cost is fair, much like the FERC makes sure that electric companies don't charge more than what's fair.

Personally, I think a central single payer system is the way to go, and that's not a wrong approach, just different.

I agree totally with the mock billionaire's point, but not their approach. When a group gets its permits and formally has a big rally, it's not right to go into the middle of it and stir up trouble. In the worst cases, it can lead to violence. If you want to mock or counter protest - fine - just don't do it from the middle of the other guy's rally. This is about respecting civil conduct during matters of heated disagreement - and that enhances everyone's freedom to speak their minds.
