To be fair, she did fall in love with him when he was just planning to be a Maths Teacher. In the book she really is put out she doesnt see him or hear from him for 2 years, but a soon as she sees him again all beaten up with his face all broken, she's overwhelmed. So ironicly in the book its because she sees him vunnerable not powerful.

At this point Kay knows Michael murdered Salazzo and McCluskey and that he will be a mafia Don. She struggles with her decision. She must be thinking "Well beng a mob kingpin's wife has some downsides but there's a lotta money in it too. OK, I'll get in the car." I also imagine that Kay had a boyfriend at ths time but that Michael bought him off or had Al Neri frame him for a crime to get him out of the way.
