the difference between medium and expert is the notes they have you play. Medium only uses the first four fret buttons, hard moves you up to all of them, and Expert has you start hitting tougher chords and solos. most people don't think there's a difference between Hard and Expert, but it's fairly evident in Bark at the Moon and Cowboys from Hell.
