When your sanity drops, you'll start to hear weird sounds like wet footsteps and maybe even hallucinate which means you'll see a monster appearing out of nowhere. The state of the portraits of Alexander are also bound to your sanity. High sanity = normal portrait, low sanity = disturbing portrait :> Hope it helped!
