This is actually much safer than running long distance without weights. Short sprints with weight will be rough, but running 10+ miles, training for a marathon for example will wreak havoc on your legs. i am a 2- time marathoner here, converted to weight and cardio training and not injured once since I changed. When I trained for a marathon I was constantly treating some leg foot ankle injury.
How is it safer with weight? With weight puts more stress on the knees and ankles and if you fall (for whatever reason) you will hurt yourself more.
The joint issue is why I haven't sprinted uphill with any more than a few pounds of water on my back. I have had enough ankle and knee problems that I don't need any more.

@StrengthFromAbove83 I can do that with 145lbs on my back. Probably more as well! Sprinting uphill with weight or no weight on your back requires balance. If people can't balance that then they should stick to treadmill workout.

It's not "sprints" it's more of a jog in this video. Not a knock on the vid btw, this looks like a VERY difficult workout.
I don't really think you could sprint safely or effectively with a backpack full of kettlebells.
Running hills is the shit. Nice video.

@BFFboss I put my 200lb girlfriend on my shoulders and do these on steeper hills, up and down. It's not quite a sprint, but so what.
