As far as I know the US has limited missile defense capabilities on standby that might be able to intercept a few missiles but would be unable to deal with the hundreds that would be launched in global war. Thanks to MIRVs missiles can hold up to 10 or 12 warheads, each able to destroy a separate city thus allowing for thousands of warheads to rain down in a very short time span. Nuclear proliferation raises the possibility that such a war will happen, which is why the US is against it.
