its called nucleation, the extremely small holes on the mento piece (similar to craters on the moon) create small bubbles of CO2 when put in the carbonated drink, each hole produces thousands of small bubbles, and it they all rise to the top of the bottle.

they dont use diet cuz its less sticky, it has a couple of ingredients that normal coke doesnt have, and those couple extra ingredients help the nucleation process
