One time in the Portal 2 co-op, I was standing where some stairs were going to be when my partner activated them. The stairs went up like normal, but instead of being on them I got stuck under them and couldn't get out! My partner put the stairs back down and back up again a few times trying to get me out, but I just went deeper in. Eventually I was completely under the floor with the stair mechanism and he couldn't see me.
