They're not really needed in this piece -- he could just as easily have used a regular lute. The exception is the last note where he plucks one of the bass strings.
This instrument was probably made from a tree, but he didn't cut the trunk, if the camera was (REALLY) farther we would see branches and leaves, I am almost sure there..

The theorbo is one of many lutes. It was developed late in the lute era and was used to accompany singing or for use as a continuo instrument. Little solo music survives for it, mostly from Italy.
