great range :p i dont believe you have to practice to get to points as such. Just gotta understand how it works. but for a good sounds, practice of course :). your high notes are great sounding. Well, it sounds like you practice too much in that area, because your " in the staff " notes dont sound too focused. but they still sounds great as well.

I use 14A4a because it is good all around to me even though I can't that high with it only a high G above the high c. I use a vinage jet tone dsp for when I want to get to the double c.

To me that looks like an asymetric mouthpiece. Good for upper register but I don't think they are that good for mid range stuff.

Your intonation in the upper register is nice, but it sounds your using a 14a4a or some other crazy mouthpeice, because your middle/low ranged intonation sucks, but i think its the mouthpiece, not neccesarily you. try to use a simple 7c or so and do that, that way you have a good, clean intonation throughout your entire range, dont be afraid to suffer a small drop in range for clear, precise tone. Overall, good job.

any slurring exercises, pick a peice of music that after playing it your face feels like puddy the peice of music does not need to be range music. the very next day take your trumpet out and play that piece of music again this strengthens your lips. PLAY YOUR TRUMPET EVERY DAY!!!! even if its only for five minutes. get the blood going in the lips. there is so much more if you need more let me know late
