In the episode "The day of the Black Sun" Zuko askeds his father where his mother is and he says that she was banished. Zuko's mother is still alive, but Azula was halusinating in Sozin's Commet.

no she was looking away from the mirror. her mom was in the mirror but it was an illusion so she could look away. and when she smashes the mirror, a ceiling veiw of the room was showed and her mom was nowhere in the room.
