Let me respond from having been "inside the tent" of the medical industrial complex. I have seen from personal experience that there is a way to map the process of care and measure the costs of such care by using tools like iThink computer modeling. Systems thinking and computer modeling of actual service delivery is feasible and will revolutionize care. But it will require more courageous leaders in the field to do it.
A mature industry whose operating cost is extremely high …. This kind of reverse process flow starting from customer is an interesting concept. I wonder if this concept can be used in other industries such as "airlines" etc. where cost of running the business is very high....?
I am actually thinking, if we can measure all the cost and benefits exactly, will the organization become more efficient? Perhaps can help improve some steps if there is a standard process. The question is "is it possible to estimate a new patient's cost" using their formula and "can we exactly calculate how much patient next year".

5) Studies from "specialists" finding out how almost no provider follows protocol correctly, so tracking them is no use
6) Studies in other places saying costs are different, since almost everything is different

Activity Based Costing with the patient as a driver in a timeline... so, as I am over 100 years old, I predict:
1) a race for consultants promissing this kind of cost study, raising the costs
2) expensive and time-consuming process tracking, raising the costs
3) Providers still not giving a s*** and trying to charge as "fee for service"
4) Studies from 'specialists' that find out these tracked processes change everytime and are difficult and costly to track
