What`s happen with Paul Pierce at 5:19 - 5:21 ?

He sustained a knee injury in Game 1 of the '08 Finals.
In that sequence, he was carried off (and later wheeled to the trainer's room) because nobody knew how serious the injury could be. Though it wasn't as bad as feared - a strained meniscus - it's likely Pierce would have done a lot more damage had he walked off.
