What i'm saying is that the bolt catch not working would likely cause the charging handle to not work, because the bolt itself is most likely jammed. In which case you would have to use the forward assist. if it was SPECIFICALLY the bolt catch that failed (which can happen when you over-lube, and it gums up with dirt under the bolt catch) then obviously you have no choice but to use the charging handle, yes.

Bolt catches do fail I guess. Its never happened to me luckily. I have to replace one for a friend when he gets a new assembly because it keeps catching on the bolt carrier so it probably has a weak spring or something similar. A majority of bolt catch failures I've seen have been weak spring or follower issues in the magazines. I wonder how the guy gets his first round loaded to begin with if he doesn't think the charging handle works.

I agree with your explaination except for calling the bolt release weak, or it breaking. I joined 9 years after Nam and was in charge of 300 M16's, and in 16 years never once had a bolt hold open fail. if it fails, then you installed a non-milspec replacement.
