Sex is what we are defined physically, simply put if you have a penis you're sex is male regardless of whether you are male or female mentally... they are different

for Hideyoshi's case, he identifies himself as male mentally (gender) but his sex is never confirmed (in the anime itself) even though he constantly says he is a guy
