It's because he cleared the stage, yet he died. The game doesn't know how to handle that since death isn't supposed to occur once the stage has been cleared. The game is trying to load the next stage but ultimately interrupted, confusing the game.

Yea, and if you press the C button a few times after this happens, you'll get things like "ADDRESS ERROR" and "LINE 111 EMULATOR" and stuff like that. It's pretty crazy.
