Almost every fast food joint (including many restaurants) have appalling conditions. The majority are filthy, nasty and usually have rats and roaches. Most places only stay open because the health inspector announces their visit a week or so ahead of time which gives them time to "clean up".
Some family run restaurants do keep their place clean. If you want to see for yourself, next time you are at a family restaurant, ask if you could get a tour of the kitchen before you order.
