I'm not an Engineer so if sombody who knows trains can chime in please do so. but if the brakes were set at that speed and if the train is fully loaded it would take a vary long time for the train to stop.

I worked in light rail for 10 years, and we had a rough interlocking at a facing point crossover with a old out of service freight siding and you'd look at thing and even though you knew you had proper indication and alignment and knew the switch on that old spur was spiked, I would cringe every time I ran over it until MOW finally got rid of it.
@kubapl90 that guy was then hired by another railroad. He remembered to switch the tracks and the train ended up ramming into a bunch of train cars on that siding XD

who left the switch open the grain train was switching an elevator and forgot to close the switch behind the train as a result the train heads onto the spur and rams the grain from behind and derails. it was the switch mans fault he left the switch open. where I live every switch that goes into a grain elevator has an electronic lock on it when the lock is turned all signals turn red and in front of the train blue flags go up and tell other trains to stay off the spur until the lock is closed.

I'm a train driver. You can hear the guys hit the emergency tap when they see the track isn't set correctly. Still gives me a shudder sometimes when you are approaching at speed and just for a second, even though you have a green light, you think "I wonder if the points are right...?"
@Organgrinder1010 the switch should have been lined back for straight track and locked when trains are traveling at that speed. Human error, I have done it in yard service but trains are moving at yard speed. It may have been someone with a key who didn't like the RR? I have been retired for years and still have my switch keys.

I don't know what it was that day but something told me to go slow in that area or else I could have been into a similar accident.
