I think it means when they write the questions and choose the answers they pick 2 specific options for the 50/50, obviously the correct one and then perhaps one that isn't too obviously NOT the correct one. I don't know if that made sense :D

what it means is even though they don't pick the two most useless options live, they try to anticipate in advance which two options would be the easiest for a contestant to rule out and choose those to be eliminated by the 50/50

It's hilarious to watch the celebrities play because they cheat. They tend to talk back sometimes and they give each other the answers. At one point Regis had to point at the celebrities who were still waiting and exclaimed "IT'S NOT.. YOUR.. TURN!!!!!" LOLOO.
