I've seen him play that lick on a seven string on which he tuned the top three strings in octaves. (This is basically what he did when he re-tuned in the video) He then left the bottom four strings in their normal tuning to play the rest of the song. So I think that he just put that 3 string octave tuning on its own neck (without the other strings) and kept a regular tuning on the bottom neck. (This way he can play the arpeggio without having to string skip too.)

I have girl hands and I can reach that far if that high up on the neck with my thumb in classical position. It's mad hard to play that lick but not because you don't have hands as big as Paul Gilbert's or 10 inch pinkies. It's hard to play because of the string skipping. You can't roll and sweep. The lick's not impossible. Just highly improbable for most of us.

But he practiced like mad for years. It's not like he just picked up a guitar and insta-shredded the hell out of it.
