You can't "get away" from a radiation burst, optimally the burst expands out almost instantly to a minimum of 5 miles from the epicenter of the blast. 10 to 20 miles or KMs depending on the optimal yield of the device. Fallout is different than radiation, fallout is contaminated particles that were carried into the upper atmosphere that then "fall" back into the biosphere.

Depending on where you are and the prevailing winds, you wouldn't even have to worry about fallout. Refugees and degradation of services would be your biggest concern.

We're not talking about a stolen warhead here; this would be basically be a 'homemade' nuclear weapon (you can actually find plans to build one if you search online); all they would need would be plutonium and the basic materials to construct the triggering mechanism, etc. If the device were simple enough, yes, all they would have to do is push a button.
