Awk why did you do that to her beautiful mouth? Great skills but seriously, big eyebrows don't mean ugly. Adriana Lima is without a doubt one of these most naturally beautiful women in the world. Don't make noses too small or they make the rest of the face look too big and the nose itself looks like a snug.

Like Joan says you can use blur, I prefer the airbrush look myself, takes a bit longer but it can make skin look very good. Just make sure to sample the spot you want to airbrush so you can keep the skin with the natural Lighting.
nice work, but i would suggest you to work rather on face which are less "perfect" because if you "makeover" faces like this they start to lose detail and depth ;) keep it up!
create a duplicate of the original layer and a duplicate of the current layer before every major change e.g. a 'hair' or 'skin' layer then merge at the end when you're happy, this will stop data loss and you can change things later on. <3

Instead of getting tired with that patch tool.. Use blur tool to make skin smoother... :)
