Sasuke knew about it because Itachi told him, since they were both Uchiha, and no, Kakashi knows how to open the gates, he just chooses not to. Also, Itachi didn't discover Mangekyou until a few days before the Uchiha massacre, when Uchiha Shisui disappeared. Shisui was Itachi's bestfriend who he killed, thus achieving Mangekyou.

Also, Kakashi did open the 1st gate. They zoom in on the brain, which is what the 1st gate does, it lifts the limit on the brain. So he obviously opened it.
