Hal Needham just talked about his new book "Stuntman". During rehearsal, his favorite horse broke his leg in a gopher hole. Hal had to shoot the poor animal himself.

This stunt was of course particularly dangerous. He had to do a 14 foot, standing long jump (which is impossible without the shoulder boost from the horses wither) over the head of one horse to the next. Falling off meant instant death.
