My best guess it that it was a high angle loose construct COTE massing about a tonne, probably a comet fragment, that went bolide at about 40000ft. It is called a fallstreak hole or punch cloud. it is caused by the water vapour in the cloud dropping to freezing point but without anything to act as a crystal nucleation (See 'super cooled water' on youtube) then when something dose act (sometimes a passing jet can ignite crystallisation) ice crystals start to form, this sets of a uniform circular chain reaction and the ice falls to the earth leaving a hole.
