The passageway is so narrow that it provides enough space for the warthog to concentrate its attack forward. So it's just a matter of perfect condition where the lion can't out maneuver the warthog (or possibly got stuck) and the warthog can focus its single forceful attack accurately.

2) How does a big healthy tusk from a huge warthog at 3:12 break off in the soft tissue of a lion's neck?

Not that I doubt this at all, but would still have loved to seen the live video. Some pigs can be extremely tough. There's a vid of 3 boars just completely demolishing a leopard who ran away from them for its life. In that video, it is clear that any ONE of the boars would have beaten the leopard.
