If you take a look at the DC-10's safety record, all the accidents after 1989 were due to heavy/bad landings, Aborted Takeoff's, or just bad weather.

The thrust reverser had broken on a previous flight, so Continental (The operators of that DC-10) used a third-party supplier who had not been approved by the FAA. It fell of during takeoff, and no one saw that strip until the investigation was well underway.
Crash stats are a little biased against the Dc10 as not too many were built compared to the 747. Both had major issues, particuarily in the 70's when jet aircraft of that magnitude was still new technology.
Dc10 as far as I know has about 30 hull losses out of about 400, but the majority were in the 70's..
Despite its troubled beginnings in the 1970s, the DC-10 ultimately proved a reliable aircraft, much loved by engineers and pilots. The original DC-10-10's notorious safety record continuously improved as design flaws were ironed out and fleet hours increased. In fact, the DC-10's lifetime safety record as of 2003 is comparable to similar second-generation passenger jets.[6] Increased inspections and modifications made the DC-10 among the safest aircraft for passenger travel.
It was rushed to production with flaws due to MD'S financial situation and competition with the 747. A few flaws were inexcusably not corrected after crashes leading to other crashes
