I copied and pasted your source code and it worked pretty well but when the email arrived to my Gmail account it said "unknown sender" and I couldn't see the originating email address. Any idea why this would happen? I know I entered a "from" address when filling out my test form.

sorry the video is so blurry. i had to convert a high resolution file to a smaller one which messed it all up. however you can download the source code.
