They had the person who sent the game (you'll hear him asking her for directions in a couple videos) translate it for them as the game progressed.

Some puzzles may need some help for the instructions, but it's mostly a self-explanatory game and even has a map system that tells you where to go next so you're not hanging in the dark.

However, for other consoles besides JP this game will not work until a translation patch comes out. Currently an English translation patch is about to be released.
