Every time there is a bank robbery in a GTA game it goes wrong. In GTAIII u have to drive 3 mobsters to the bank and you hear gunshots right away. In Vice City, half of the crew dies. In San Andreas the alarm is tripped pretty fast. And in GTAIV Michael dies.

Any that's left and that you can afford. I usually buy the Malibu Club as the last asset, because it costs so much.

One asset that's a good investment is Sunshine Autos. When you have bought that asset, you can start doing street races and the longest street race will net you $30,000 (if I remember correctly) if you win. That's an easy way to earn money to buy up the other assets.

No, Cam doesn't always die. You can actually escape with him alive. If you do keep him alive you will get a little different dialogue at the end of the mission, but that is all.

I bet there is a money cheat, but I didn't use it. One very good way of earning money that I often use is by doing the street races at the Sunshine Autos asset. Doing the longest street race will give you +$30,000 each time you win.

I maybe have one of the easiest way to earn much money in vice city.

I had 16 000 000.

You search for a tank, you could spawn them with a cheat or you find those hidden packages. Then you start doing those missions. At some moment you earn 64000 a mission. It's very easy to earn alot of money.

What I really like about this game as opposed to the other games in the series is that Tommy is actually building a criminal empire, getting missions specific to the assets he acquires. Sure, the other games have elements of that, but it's not been done this well in any of the other games IMO.

It doesn't matter if Cam lives or dies when you exit the bank - you can still complete the mission. However, depending on whether Cam lives or dies you will get different diaglogue at the end of the mission.

I have about 2000 ammo for the submachine gun, which is normal for me. If you don't have that much ammo buy som more at AmmuNation. Money is easy to get if you do some street races, so that's no problem at this stage in the game.

You get 150 hp by completing enough levels of the Pizza Boy mission.

You get 150 armor by completing enough levels of the Vigilante mission.

Yes, you must save Phil Cassidy from dying, but you can also save both Phil Cassidy and Cam Jones. If you save them both you will get a slightly different dialogue at the cutscene at the end of the mission than if you only save Phil Cassidy.

No, you actually don't have to buy the Malibu Club (or do the Malibu Club missions for that matter) to complete the main storyline. However, if you don't do the Malibu Club missions you won't unlock the Phil Cassidy missions, so you'd be missing out on those.
