22:30 has the explanation. The water level at the station was at the highest level, the 2nd generator was slightly off-centre and during the previous night it was put into full capacity. This led to rapid corrosion of the mountings which eventually pulled the turbine apart. Everything happened in 7 seconds.

Before Panama was Vietnam and we know what happened there.
