If You think about it Sounding the Seventh Trumpet Is pretty much the same like Waking the Fallen. In Sounding the Seventh He Screams and Sing. He doesnt only scream. for example this song, Streets, Turn the other way,Warmness on the soul, THe Art of Subconscious illusion. They either do half singing or all singing songs like Waking.
if you were a real a7x fan youd know that b4 they made waking the fallen they already decided he didnt wanna scream any more so they did a half screaming album (Wakin The Fallen) and then gave it up
The surgery was for a burst blood vessel in his neck. he can definately scream just as well as before if not better, but it was a band decision to stop the screaming, also if he did continue screaming their is a chance his condition in his throat could get worse.

if you don't believe me, you can look up critical acclaim live at the key club, he does some pretty cool screams there
