it could still be derek because if you look at the point where derek might be standing between bailey and the chief, it could be a really fast cut of two clips in the show. it kinda looks like that since its not a flowing transition between the two shots of bailey and the cheif... its like two different shots.

you're both right. It is Derek on the bed, but Derek is not the man that the chief is referring to when we says "we're saving this man". On the other hand, the main point is not whether Derek has an accident or not, but whether he survives it! (and also there's that theory about it being a nightmare).

it's 100% freaking alex!!! if you stop at the 0:08, you can see someone being revived through CPR. If you look at a picture of Justin Chambers sideways, the shape of their heads are identical and the nose pretty much gives it away. Plus, it's not Derek because while the chief was saying we need to save his life, Derek was right beside him. Also to Rose, she should just admit that McDreamy doesn't love the rebound girl. She's desperate.

I think it IS Derek. It is definitely not Alex because their noses are completely different(I have a thing with noses and it's definitely Derek's!) I think that clip we see of Derek is probably from early on in the episode and later comes Derek in the strecher. And I don't think the patient in the strecher is the same patient that the chief is talking about(he must be talking about Callie's patient). I think it might be a dream but I know it's Derek. Why else would Meredith be screaming?

Ok, i rewatched this over and over... It doesn't look like Derek in the gurney, cos if you watch it again... Derek is standing right beside the Chief when he says that we have to save this man. But, you know who it does look like.... Meredith's father!!!!!! Thatcher. That could be why she is screaming! And i think the whole rose pregnant thing is a plot to stop Derek from breaking up with her. The first time i watched this i completely freaked out and started crying!!! What does everyone think?

I honestly don't think that's Derek. If you watch VERY carefully, right before Richard says "We are saving this man" Derek is standing right next to him. Also, Shonda has said that Rose will not be pregnant, and Derek will not get into an accident, and such like that, and several other spoiler sources have confirmed it along the way.

2)We see Derek near Richard but he is smiling. He is too smiling to be in a tragic situation like that. Could he not be out of himself and meeting Denny?

3) Just focus on the appearance. At first sight I'd have sworen it was Derek. Now he could be Alex too even if it had less sense.

4) Meredith's nightmare? Maybe.
