To sneak up on guards, you need to use a new sneaking control. They'll hear you're footsteps even with the slowest walk. Walking with the D-pad is a silent but slow creep that you can use. The guards patrols have been made slower to help accommodate the new creeping control.

@hardstylekid123 Sokolov tells Snake in the previous mission that he speaks execllent Russian despite all the dialogue in the cutscene being heard spoken in English. Basically, the language the player hears isn't necessarily the language that's being spoken.

Dr. Clark was originally implied to be male in the original Metal Gear Solid. In the English version, Naomi repeatedly uses the male pronoun "he" to describe Dr. Clark, while in the Japanese version Naomi only uses the equivalent pronoun kare once. As others have pointed out, use CQC. Most of the time when I played I didn't rely on the tranq to knock people out.
