we have seen this on a few xtr cassettes that have come into the workshop, the main cause was incorrectly (under) torqued cassette retaining nut .

no, there no ways the cog inner teeth broke off i say its the freehub

Looks more like a freehub body error unless the cog's inside splines broke off.
