He was horribly burned by some thugs who busted up his lab where he was working on artificial skin. The treatment for his burns included making him hopped up on adrenaline all the time, so he's super strong and super angry. He was going out with his girlfriend who had thought he was dead, and then loses it on some carny who wouldn't pay out the prize he won.

After the finger twist, Peyton tried to give the bear to his gf, and then his mask began to run out of time.

He was so badly burned earlier in the movie that he had to have his nerves completely severed so as to not live the rest of his life in unbearable physical pain. As a side effect, his mind ends up focusing on his emotions full-time leading to violent mood swings and unchecked adrenal levels (where the "Hulk"-like strength kicks in).
