Thats interesting, hand sewing the Gastrojejunostomy. I work with a surgeon who puts vicryl to each corner of the jejunum and sutures it to the stomach then takes a white or blue load on the ethicon stapler and fires it then uses 3-0 PDS to close the opening and then uses the SurgAssist. This is a very interesting video Doctor!

This is amazing. Have the manufacturers of the instruments ever thought to put an inch or mm scale on the clamps themselves? I saw a measure tape used inside. Thank you for helping me see what she had done. Its all amazing can't believe how well you do the stitches and tie offs. Amazing work Dennis.

@flashanddancedj - yeah I'm an RN at a private hospital, I'd just been YELLED at by a gastric bypass pt' on night duty for doing my job as she was sick of me as she puts it 'annoying her' - I foolishly came on yt to vent' - Sorry 'bout that. I deal with these people EVERY DAY...!!! If I had my time over again I'd be a housewife with at least three kids, either that, or be married to a Colombian drug lord, sippin' pina colas on a yacht on the Caribean...

Great Job!!! My wife literally had this done Yesterday at 7:20am and as of 7pm today she finally was able to advance to stage 2. You are very very skilled at what you do. This was NOT robotic. I do not use the robot - it is not helpful or necessary in the vast majority of cases. This is done entirely with my hands (and my outstanding assistant's hands!) and the laparoscopic instruments.
Yes, it seals the staple line so that there is less chance of micro-leaks, which could potentially turn into larger and significant leaks. It also helps ward off ozzing of blood from the staple line.

Thanks for your compliments! I do in fact feel blessed to be able to help people who are in this situation. The Fobi pouch is just another way to do the gastric bypass, but it involves the use of a ring around the pouch. I don't like the idea of a ring - it is very unforgiving, and many surgeons feel it leads to more vomiting issues. It is also a foreign body, which I like to avoid, since there is always the chance or erosion of foreign material into the pouch.
Dr Smith, what do you think about the mini gastric bypass(MGB)surgery? Is it an overall(including less medical risk)better bypass surgery than the Roux-en-y as I have been told it is?
PS MGB is only about 17k too! :) I'm interested in it but need more information.
The "mini" gastric bypass is basically a Billroth 2 operation, which was abandoned many years ago because of concerns over bile reflux into the stomach potentially causing cancer many years later.
The vast majority of bariatric surgeons denounce the MGB for weight loss because of these concerns. The statement that it's less risky comes from MGB surgeons, of course. There are many complications and reversals of the MGB that happen at other hospitals.

Dr. Smith did this surgery on me December 20, 2006. I am so pleased with the results and thankful to Dr. Smith. He is a wonderful surgeon.
