That picture thing scared the crap out of me in my first playthrough. It happened in the room next to the "dog room". I looked at the painting in the room then opened the cabinet which made bones fall out and I lost sanity. All of a sudden when I look back at the painting that creepy-ass face is there instead. Creeped.....me.....out!!! Was the first time I saw that.

I open the drawer to the desk, when suddenly a pile of bones fall out, making me (and Daniel) scream out like a sissy girl. A similair reaction followed when I turned around and looked at the painting again.

You don't need to go to the torture rooms in order to see the creepy painting, you just need to have the sanity at the lowest level possible (Which is when it says "..." and the screen gets all blurry and the character is hard to move)

Amnesia is a horror survival game. Your character is afraid of darkness.

When you stare on enemies, stay in darkness,... You sanity level will decrease and your character will hallucinate. That's what happened to the portrait.
