yes, it is shady jeff screaming in this song, you can tell the difference between his screams and Da Kurlzz's, but really the guy knows he has it for the wrong person, so no need to go on about it. and it is actually Charlie singing "I'll hold on tightly" that line sounds so much cleaner than Deuce's parts right before and after, besides, Charlie can sing (Bullet), plus if you listen really closely in other songs like Pain and Black Dahlia you can hear quieter singing in the chorus thats him.
