i think that since the skateboard broke in two pieces drifting rapidly in two different directions, his skin in his groin area did somehow break / was teared apart. probably not at the balls/scrotum though, more likely where the leg ends so to speak. shouldn't be to difficult for the meds to take care of

another option is that some leg or pelvis bone split off and went into scrotum, which is very unlikely, but would be extremely painful
