Not really. Once you get rid of the purely gameplay features (Inventory, health, karma, reputation, etc) you have a device that is actually quite primitive. GPS and a radio really.

Now if you wanted to have one with all the features of an in-game one... well then you'd have some trouble. Health would be the only one remotely possible, and would involve sticking needles into yourself. Then you could auto-inject stimulents and painkillers

Only in development costs... and legal ones if the "medical" features malfunctioned, and lets face it: Trying to replicate the medical features of a pip-boy would be a horrible idea. You'd basically have a button on your arm that gave you hits of morphine.

Other than that though everything is easily modern tech. I could scotch tape a iPhone to my arm and have better functionality than the mere radio-gps functions of the Pip boy. Would be clumsy as fuck to wear though.
