You don't hear the guitar clicking at all. what you hear is the in-game sound. The person used a digital recording tool, possibly a DVD recorder to record him playing. Thats why you can't hear him clicking.

It is possible. Having an FC at this song for a normal person requires something like 3 years of guitar hero or more if you are not talented. Altough some people may do it after 6/8 months of guitar hero id they are really gifted
