Here we go... let's apply that logic to EVERYTHING! A diet of water is just about as healthy as a diet of heroin therefore don't drink water. A diet of celery is just about as healthy as a diet of heroin therefore don't eat celery.
It's an advert - it makes a fucking analogy between unhealthy addictions and uses the massively unhealthy image of one to make a shocking contrast with a another unhealthy habit.
Like the anti smoiking advert which has grease oozing out of cigrattes while people smoke - it was making the point that smoking clogs arteries. Cigarettes don't contain fat.

The message is clear, trivial really: better to avoid feeding junk food to your family. What's wrong with that? Nothing, unless you're in the pay of the fast food industry (ie a corporate imperialist shill) and more concerned with maximizing profits then your customer's health.
As for the heroin/burgers switch, that's an attention hook/device. Only complete morons would take it literally.

1. Humor: A personal OPINION thing, useless to debate. Some saw humor, others didn't. End of story.
2. Literal view: Summarizes every negative response dispite the obvious fact that no one was killed and no one was advocating murder. See 1.
3. Exposure: Dispite everything 1010 got world wide exposure
4. Abortion: Swapping in abortion (ending life) dishonestly (typical HTWW) changes context, a dead kid is really a dead kid
