The pulling part didn't hurt. But when the girl's mum was tying the first knot with a lot of force at 0:20, that must have hurt a lot cos the tooth was being pulled towards her mum and not downwards, or the string must have cut her gum. That's why the little girl was screaming with pain.

you have to tie a knot in the dental floss, then slip it on the tooth, make sure it goes all the way up to the gums, try to get it under the tooth, then tie a double knot so it don't slip off.....then pull down or up where ever the tooth is!
